MediScribe Flow is a clinical documentation workflow tool. This page explains how it is intended to be used and the responsibilities of the practitioners and organizations that use it. Please read it alongside our Terms of Service and Privacy Policy.
A documentation aid — not medical advice
MediScribe Flow helps practitioners capture, structure, and review visit documentation. It does not provide medical advice, diagnosis, or treatment recommendations, and it is not a substitute for the professional judgment of a qualified practitioner. Nothing the Service produces should be relied upon as a clinical or coding determination on its own.
Not an electronic health record (EHR)
MediScribe Flow is not an EHR and is not a system of record. It does not automatically submit anything to Medicare, CMS, iQIES, WellSky, or any other system. The practitioner or organization decides what is finalized and where it is submitted.
The practitioner is responsible
The licensed practitioner remains solely responsible for clinical judgment, for the accuracy and completeness of the documentation and any OASIS-E2 coding, and for the final content that is delivered or submitted. The Service is designed to keep a human in the loop: it surfaces suggestions with supporting evidence, leaves unsupported items blank or flagged for review, and requires the practitioner to review before anything is finalized.
Review before use or submission
Always review the documentation the Service produces before relying on it, delivering it, or submitting it to any payer, agency, or third-party system. Suggestions are drafting assistance only and may be incomplete or incorrect; you are responsible for correcting them.
No guarantee of reimbursement, coding accuracy, or compliance
MediScribe Flow does not guarantee any particular outcome. We do not guarantee that use of the Service will result in reimbursement, that documentation or OASIS-E2 coding will be accurate or accepted, that a record will pass an audit or survey, or that any regulatory requirement (including CMS Conditions of Participation) will be satisfied. References to "OASIS-E2" describe the data set the review is oriented around; they are not a claim of certification by CMS or any other body.
Third-party services and EHR/portal integrations
Optional integrations (for example, a WellSky integration that enables a "previous visits" feature) connect to systems operated by independent third parties. MediScribe Flow is independent from those providers unless a formal partnership is stated in writing. Third-party portal availability, data, workflows, and requirements may change at any time outside our control, and we do not warrant their availability, accuracy, or fitness for any purpose. Not for emergencies
MediScribe Flow is not designed for emergencies. If a patient needs urgent care, follow your organization's clinical protocols and contact emergency services. Do not use the Service to communicate time-sensitive clinical needs.
